Finishing your intake ahead of time means your first appointment is spent on the evaluation, not paperwork. New patients complete a single online packet — health history, dental history, privacy forms, and the sleep questionnaire, all in one place.
Which packet to complete
Choose the one packet that matches the patient and how care is being paid for. Each takes about 10–15 minutes and can be finished on a phone, tablet, or computer.
Adult — With Insurance
For adults starting treatment who are using dental or orthodontic benefits. Have your insurance card nearby.
Adult — Without Insurance
For adults paying directly or setting up a monthly payment plan through the office.
Child — With Insurance
Completed by a parent or guardian for a child or teen covered by dental benefits.
Child — Without Insurance
Completed by a parent or guardian when no dental benefits apply to the child's care.
Every packet includes the welcome form, medical and dental history, HIPAA privacy acknowledgment, and the sleep questionnaire — so one packet is all a new patient needs. If you are not sure which one applies, call the office at (817) 581-8881 and the team will point you to the right packet and confirm what else to bring.
